change より「過程」に注目する語
change は広く「変化」を表しますが、transition は「AからBへ移っていく過程」や「移行期間」を強調します。仕事・教育・制度・技術など、少しフォーマルな場面でもよく使われます。

よく使う形
名詞では transition to ...（〜への移行）、transition from A to B（AからBへの移行）がよく使われます。動詞でも transition to ...（〜へ移行する）、transition from A to B（AからBへ移行する）と言います。
よくある間違い

正: We made a transition to the new system.
「〜へ移行する」は transition to ... が自然です。for では方向が分かりにくくなります。

happen は自動詞なので受け身にしません。was happened ではなく happened と言います。
