数えられる場合と数えられない場合
「失敗という状態・経験」は不可算名詞として使われることが多く、learn from failure「失敗から学ぶ」のように言います。一方、具体的な失敗した計画・人・物は可算名詞で、a failure や several failures のように使います。

failure to do は「〜しないこと」「〜できないこと」という意味で、ややかたい表現です。Failure to pay on time may cause problems. は「期限内に支払わないと問題が起きる可能性がある」という意味です。
